Thrive Behavioral Health practices comprehensive, whole-person integrated care that assesses and connects clients with the services and supports they need to achieve optimal health. Integrated Health Homes are designed to help clients navigate the health care system and connect them with access to primary care, behavioral healthcare, substance use treatment, and community support.

Thrive Behavioral Health operates multiple Integrated Health Homes in Rhode Island. They consist of teams of professionals, including family and peer recovery specialists, who work together to provide whole-person, patient-centered, coordinated care for adults living with serious mental illness. 

To be eligible to receive Health Home care, an adult must be a beneficiary of Medicaid and have been diagnosed with a serious mental illness such as Schizophrenia, Schizo-affective Disorder, Bipolar Disorder, major depression or other serious mental health conditions that significantly impairs their daily functioning. 

Thrive's staff will assist clients with their enrollment paper work and help them to apply for any benefits for which they qualify. Once enrolled, participants will be assigned a case manager who will serve as a single point of contact and coordinate clients' access to care including therapy appointments, substance use treatment, and peer recovery support. Case Managers will ensure each client has access to health care and social services to help them improve their quality of life. Thrive's staff also advocates on behalf of clients and ensures they receive the highest level of care.
