Community Support Services

For individuals 18 years and older experiencing symptoms of serious and persistent mental illness who require daily living assistance.

Thrive’s Community Support Program (CSP) is a coordinated system of care designed for adults aged 18 and older who are struggling with a severe and persistent mental illness and require a higher level of care. CSP participants have received a qualifying diagnosis of a psychotic disorder, mood disorder, obsessive compulsive disorder and have had difficulties engaging with treatment.This program enables individuals to maintain psychiatric stability, increase their quality life in the community, and ultimately achieve their own personal recovery. Qualifying clients will have access to psychiatric services, counseling, case management services, education, peer support, residential services, hospital liaison services and transportation to and from medical appointments.

Thrive's Community Support Program is comprised of three integrated support teams including the Assertive Community Treatment (ACT) Team and Integrated Health Home (IHH) Teams. An Outpatient level of care is available to those who have similar needs but require less intensive care.
