New clients should bring the following information to assist with the registration process.

  • Valid photo identification
  • Proof of insurance
  • Proof of residency (e.g., utility bill)
  • List of physicians
  • Current medications and doses
  • Names of other facilities where services have been received

Thrive will not turn anyone away who seeks treatment. If an individual does not have insurance, there are several grant-funded options for which they may be eligible. These include programs for individuals who have been affected by a traumatic event.

Please allow 20 minutes for registration with a Health Information Registration Technician and 1-1.5 hours to be assessed by a therapist. The assessment will determine what behavioral health programs and services would best suit the client’s individual needs, and an initial appointment will be scheduled.

Thrive's Intake Service is located on a bus line and is situated next to Kent Hospital. Thrive offers free onsite parking and alternate transportation is available under certain circumstances. For more information or to schedule an appointment, contact 401.732.5656.
