Healthy Transitions

Wraparound services for individuals ages 16 to 25 living with serious mental health concerns.

The transition period from adolescence to adulthood can be challenging, especially when facing new roles, responsibilities and expectations for independence. Healthy Transitions provides a non-judgmental and encouraging environment in which to understand and work on symptom management at one's own pace.

Connected

Not only will participants be connected to their service providers, but they will also feel connected to other participants in the program.

Accepted

Participants make their own decisions without fear of judgment.

Self Directed

Participants create their own personalized treatment plan with the help of the team. They work with as many or as few members of the team as they choose.

The Program

The Healthy Transitions Program offers access to clinicians, a wraparound case manager, nurse, psychiatrist, substance abuse specialist, employment and training specialist who work as a team to assist participants with meeting personal goals for improving mental health and making successful transitions to adulthood. It includes:

  • Help with reaching goals such as housing, employment, education, family support, and substance use treatment
  • Medication treatment if mutually decided by the participant and the doctor

The Healthy Transitions Program is funded in part by the Department of Behavioral Healthcare, Developmental Disabilities and Hospitals through a grant from the Substance Abuse and Mental Health Services Administration.
