The transition period from adolescence to adulthood can be challenging, especially when facing new roles, responsibilities and expectations for independence. Healthy Transitions provides a non-judgmental and encouraging environment in which to understand and work on symptom management at one's own pace.
Not only will participants be connected to their service providers, but they will also feel connected to other participants in the program.
Participants make their own decisions without fear of judgment.
Participants create their own personalized treatment plan with the help of the team. They work with as many or as few members of the team as they choose.
The Healthy Transitions Program offers access to clinicians, a wraparound case manager, nurse, psychiatrist, substance abuse specialist, employment and training specialist who work as a team to assist participants with meeting personal goals for improving mental health and making successful transitions to adulthood.
