New clients should bring the following information to assist with the registration process.

  • Valid photo identification
  • Proof of insurance
  • List of physicians
  • Current medications and doses
  • Names of other facilities where services have been received

Thrive will not turn anyone away who seeks treatment. If an individual does not have insurance, there are several grant-funded options for which they may be eligible. These include programs for individuals who have been victimized by crime or by a traumatic event.

Please allow two hours for the initial appointment. Your assessment will determine what behavioral health programs and services would best suit your individual needs, and your next appointment will be scheduled.
